Assumption of Risk

I, the undersigned parent or legal guardian of the above-named child ("Participant"), understand that participation in Little Leapers activities involves physical movement and exercise, including but not limited to gymnastics-style activities, obstacle courses, balance activities, tumbling instruction, games, inflatables, climbing equipment, camps, preschool classes, birthday parties, country club programs, and special events.

I understand that participation in these activities carries inherent risks, including but not limited to falls, collisions, strains, sprains, bruises, cuts, fractures, and other injuries that may occur despite reasonable supervision and safety precautions.

I voluntarily assume all risks associated with my child's participation in Little Leapers programs and activities.

Medical Authorization

I certify that my child is physically able to participate in Little Leapers activities and that I have disclosed any medical conditions, allergies, behavioral concerns, or limitations that may affect participation.

In the event of an illness, injury, accident, or medical emergency involving my child, I authorize Little Leapers and/or the facility hosting the activity (e.g., preschool) to seek and obtain emergency medical care for my child if circumstances reasonably require prompt medical attention.

I understand that all costs associated with medical treatment, transportation, hospitalization, or related care are my sole responsibility.

Release of Liability

To the fullest extent permitted by law, I hereby release, waive, discharge, and hold harmless Little Leapers, its owner, instructors, employees, contractors, volunteers, affiliates, host facilities, schools, preschools, camps, churches, country clubs, event venues, and property owners from any and all claims, demands, causes of action, liabilities, damages, costs, or expenses arising from or related to my child's participation in Little Leapers activities, except to the extent caused by gross negligence or willful misconduct.

This release includes claims arising from emergency response actions, first aid, transportation for medical treatment, or other actions taken in good faith to protect the health and safety of the participant.

Property Damage

Little Leapers is not responsible for lost, stolen, or damaged personal property belonging to participants or their families.

I agree to be financially responsible for any intentional damage caused by my child to Little Leapers equipment or venue property.

Illness & Participation Policy

I agree not to bring my child to Little Leapers activities if they are experiencing symptoms of a contagious illness, including fever, vomiting, diarrhea, or other symptoms that may pose a risk to others.

I understand that children must follow instructor directions and participate in a manner that promotes a safe environment for all participants. Unsafe or disruptive behavior may result in removal from an activity without refund.

Parent/Guardian Acknowledgment

I acknowledge that I have carefully read this Waiver and Release of Liability, fully understand its contents, and voluntarily agree to its terms on behalf of myself and my child.

I understand that by signing this document, I am giving up certain legal rights, including the right to bring claims against the released parties for ordinary negligence.

Electronic Signature Consent

I agree that my electronic acceptance of this waiver, whether through a website form, registration platform, email, or other electronic means, shall have the same legal effect as my handwritten signature.
